In order to make our time together more efficient, please download the four forms below and fill them out. The Consent Form, Intake Form, and maybe the Release of Information Form need to be filled out. The HIPAA form needs to be read. If you have recent labs, you can provide those as well. There are instructions on how to deliver the forms to Dr. Allott in the Welcome and Consent Form.
